 How to report 401k distribution???
My husband withdrew his 401k and must report it as income on his 1040. How exactly should it be reported? Is there a specific schedule that needs to be added to the return or list it as "other income"? Please assist.. tax forms are overdue..

All line numbers are for 2002:

Form 1040, line 16 (Pensions and annuities).

If a penalty might apply, form 5329 or possibly directly on form 1040, line 58, if the 1099-R correctly reports code 1 (early distribution) and there are no exceptions or exclusions from the penalty.
